The horizontal/vertical dimensions of this adapter are correct, but the adapter itself is too thick to fit into the SIM slot for all the phones I tried. Specifically, it doesn't fit in an iPhone (2G) SIM tray or into any phone where you have to slide the SIM card under a metal bar and into place.
If it works in your phone, it'd be great, but it ended up being totally unusable for me. (I wasn't completely surprised, since other reviewers had made a similar observation.) I figured it was cheap enough to give it a try. Unfortunately, it didn't work for me.
(However, delivery was very punctual, etc...so my low rating is not about the seller's service at all.)

It came in a small plastic Ziploc bag (about 3 inches by 1 inch). There was a single piece of paper (probably 2 inches by 3/4 inch) with about 4 lines of directions on how to use this. It comes with a sticker that you peel and put on top of your existing regular SIM card and you cut the SIM along the dotted lines on the sticker. If you are really really careful and put the sticker properly aligned on your SIM, then you are good. If you put it even half a centimeter skewed, then forget about peeling it off. It's really hard to peel it off and put back (won't stick properly after you have peeled it off). Anyway, I put it on correctly the first time (lucky me) and I was able to cut the SIM along the dotted lines. Make sure you cut on the outside of the dotted line and not inside, because if the SIM is cut too small it could fall off the adapter and into the phone itself (getting jammed up nicely). All in all, if you are a risk taker like me and don't want to spend $30 to get a sim card cutter (again like me) then go for it.
After I cut the SIM I can now use it on my iPhone 3gs (with the adapter) and on my iPad (after changing the APN settings to phone) as well.

I've purchased this adapter to put my iPhone 4 Micro Sim card in and use with my Blackberry bold. My job requires me to use a Blackberry when I'm traveling but when I'm home I love my iPhone 4. If using AT&T - this little adapter is priceless! Make sure you get the Blackberry data plan, modify iPhone APN to use WAP.CINGULAR and you can successfully swap sims back and fourth. Also works great for iPad!

I just switched from an iPhone 4 to a Galaxy Nexus and needed a way to reliably fit my micro-sim into the full-size sim slot that the GNex has. For the past few days I just shoved the micro-sim into the holder hoping that it wouldn't flop around.
For a more permanent solution, I bought this adapter which is simply a very functional piece of plastic. It holds the micro-sim just like you would expect it to.
I cannot comment much on how good the template is, but I would assume it lines up with the SIM properly and makes cutting a micro sim much easier (I tried making one on my own and it was a pain).

This is the flimsiest piece of plastic I've ever seen. Once it was in it was impossible to pull it out again without breaking it. I ended up using tweezers and it still cracked immediately. A dollar was even too much to pay for this. Spring for a better one, trust me.
